COVID-19 Updates
We know these are challenging times for you and your loved ones. Like you, our utmost concern is for the health and well-being of each of our patients and to ensure our staff is healthy and able to continue to care for patients. With that being said, we continue to take all necessary cautionary measures and are strictly adhering and implementing all CDC recommendations and guidelines. We continue to also work with local and state agencies as we fight Covid19. During this time we also recognize how important it is to keep the lines of communication open to give you peace of mind while we continue to meet the care needs of your loved ones while at our facility. As such, the facility will be providing updates of new developments as they take place on this page to keep you updated.
Scheduled Window Visitation Update
November 30th, 2020
This morning we found out that a 3 residents and 4 staff members tested positive from last week's testing. We communicated this to Los Angeles County Department of Public Health and will work with them to ensure we stay on top of the situation. We have moved our positive residents to the COVID Red Zone and our positive staff have initiated their quarantine/isolation.
November 27th, 2020
This week we are thankful that all the residents and staff we tested last week resulted with negative COVID-19 tests. We tested again this week and are awaiting for the results of these testing efforts.

November 13th, 2020
As Our efforts continue to ensure everyone in our facility stays safe, we are back to our screening mode of testing. We are glad to announce that the residents and staff we tested last Friday (11/6/2020) and this week all have returned negative COVID-19 test results. Be assured of our continued effort to work with and obtain guidance from Public Health Department.

As we mentioned on last week's COVID update we have been working with Los Angeles County Department of Public Health in developing a visitation plan. With their input and guidance we have been able to come up with a plan. Below you will see the visitation rules. Your cooperation is of utmost importance in keeping yourselves, our residents and us safe.
Scheduled Window Visitation Rules
Scheduling appointment:
Visits will be scheduled on the following days Tuesday, Thursday, and Sunday. (may be subject to change)
Two slots will be available on each day. 11:00am and 2:00pm. (may be subject to change)
All window visits will need to be scheduled in advance. Please call our main line at 626 445-2421 and schedule visits only with Lyna Williams,Activities Department. You may call Lyna between 10am to 2pm Monday through Friday to schedule visits.
To ensure all residents are entitled to family visits, visitation is limited to 1 visitation on a month for each resident unless there is availability.
Only residents in the Green Zone (COVID negative, post 14 day quarantine) will be allowed window visitation.
Pre-visit Rules:
Before coming to the facility please pre-check yourself, if you are experiencing any signs or symptoms of COVID-19 cancel your appointment.
Please arrive 10-15 min before your appointment to ensure screening is done effectively and does not interfere with your 15 minute allocated time, if you arrive late your appointment will be cancelled and rescheduled upon availability.
Call the facility at least an hour prior to visit to confirm your appointment.
If resident develops any sign or symptom of COVID -19 on the day of visit, visit will be cancelled.
Check in:
Visitor must arrive wearing a mask and keep a mask on at ALL times during your visit.
Check in will be at the back patio.
Please follow signs and instructions at all times.
Upon arrival you will be asked to sign in and answer a few COVID-19 screening questions.
Temperature check will be taken by a staff member.
Visitation Rules:
Resident and visitor will communicate via two-way radio unit provided by facility only,
Visits will be allowed for 1-2 family members/visitors.
Only one resident visit can occur at a time.
Visitors will stay outside the patio with locked sliding doors as separation.
Visitor is required to wear a mask at all times.
A staff member will be monitoring the entire visit to ensure the safety of the resident.
Staff member must wear PPE at all times.
Resident is highly encouraged to wear a mask or face covering however if their underlying cognitive or medical condition prevents them from doing so, they cannot be forced to.
Visit will be 15 minutes maximum.
Facility will disinfect the areas used by family and residents thoroughly. Disinfecting all chairs, sliding glass window, equipment used for communication, tables, and all surfaces touched per CDC infection control guidelines.
Facility will must allow at least an hour after each visitation for enhanced cleaning.
Resident and family may not hand or exchange any items from inside or outside of the facility.
Facility may stop visitation during an outbreak or when it has assessed a risk that may endanger our residents and staff.
Post Visit:
If the visitor becomes ill or tests positive for COVID-19 within 14 days after your visit, you are required to notify the facility immediately.

Today we tested some symptomatic residents and staff using our rapid testing systems.
From these tests 2 more residents and 1 staff member tested positive.
December 5, 2020
As we continue to receive the results of our COVID-19 testing for the week, 1 more resident tested positive and a couple more staff members. We will continue to follow the guidance of our health department in mitigating this situation
December 4, 2020
Due to the Positive results we received this week we initiated a twice a week testing for our staff and a once a week testing for all our residents. The results of initial round of testing yielded 5 more positive residents and an additional 3 employees. We are continuing to coordinating with the Local Health Department and will update you with any more developments.

As for this week's testing, we are pleased to inform you that the results from our testing efforts have yielded all negative COVID-19 results. We continue to obtain updates and work with our local health department.
November 6, 2020
Today we received this week's testing results. From this testing effort we had an employee test positive. We have communicated this information to our local health department. Of the residents we tested in the beginning of this week no one tested positive among those whose results we have received.
Today we implemented a mass testing of residents and staff. We will update you further once we receive our results.
October 30, 2020
Test results for the majority of our staff have come back from this week's round of testing and so far all have returned as negative. Residents who have tested have had the same results. We continue to get updates and follow guidance from Public health.
We are continuing our scheduled outdoor visitations. If you need help with these, please call Lyna of our Activities department M-F between 10 to 3pm.

We completed our second round of mass testing week and are again very pleased to announce that everyone had a negative result. Moving forward we will be following the screening testing phase and will continue to test our staff and some residents.

October 16th, 2020
As you were updated earlier this week regarding an employee testing positive, we have responded by testing our residents and staff at our facility and following the health department's guidance. From the response testing efforts this week, we are pleased to report that no one has tested positive for COVID-19. We will continue to test weekly to ensure that no new cases arise.
We are planning to resume our window or face-to-face socially distanced visitation shortly. We will inform you as soon as we have established the resumption date.

October 12th, 2020
Over the weekend we got news that one of our staff members tested positive. This has prompted us to initiate a facility-wide testing of all residents and staff once again. We will continue testing under the response mode until we are able to determine that we are clear.
For the safety of everyone involved visitations will temporarily be on hold until we receive further guidance from the Los Angeles County Health Department.
We will be in touch as soon as we obtain further guidance.
October 9th, 2020
We are continuing to follow guidance from our local health department regarding COVID-19 mitigation and prevention. This week we continued with another round of testing our residents and staff. We have received a majority of our results and we are happy to say that all results we have received so far have been negative for COVID-19.
This week we are starting to allow scheduled in-person socially distanced visitation. If your loved one, our resident and yourself choose to visit with each other face to face instead of our scheduled window visitation, please inform Lyna from our Activities Department. With this we ask that you follow the following rules:
Please continue to follow the rules for the window visitation.
In addition,
Please socially distance( 6ft or more separation between visitor and resident)
No physical contact is allowed at this point.
No one is allowed to exchange items with each other at any point in time during the visit.
Limit visitors to 2 individuals maximum.
The facility reserves the right to modify visitation depending on the facility’s identified risk, facility’s outbreak status, community positivity rate, guidance from the health department and other reasons where the facility deems it necessary. We will make an effort to inform you as soon as we are able of any changes.
Let us please continue to use plenty of caution when doing the visits and follow the allotted time so that our staff can effectively disinfect the area.

We continue to do our weekly testing with residents and staff. We are still awaiting results from this weeks testing. We will provide updates as we receive them.
As things are changing very rapidly, we will communicate with you any changes in guidance as it relates to our facility and our residents.

September 25th, 2020
The facility continues to conduct weekly testing for residents and staff. We have not received all of our test results this week. Results that were received this week have been negative. We will provide you with further updates as it becomes available.
Residents in the facility's green zone who want to participate in scheduled socially distanced group activity will start these activities today. We will be following CMS and California Health Department's guidelines for these activities.

Testing this week has yielded no new positive cases.
We continue to get updates from CDC, California Department of Health and Los Angeles County Department of Public Health to help us implement the necessary safety precautions to appropriately care for your loved ones.

September 11, 2020
We continue to provide testing for residents and staff per the recommendation of our local health department. We have not had any new cases of residents and/or staff test positive for COVID-19.
If you are scheduled to do the window visit this weekend and on please wear protective gear to avoid any issues with the smoke and ashes while you are doing your visit. If by any chance you are not able to make it to your assigned time please let us know.

With the majority of our test results from this week's testing back, we are pleased to say that so far all results have come out negative. If all the results from this week's testing effort come back negative, we will be back to surveillance testing next week. We also started our scheduled window visitations this week and we are pleased with the feedback so far. From our testing this week no residents or staff tested positive. We will continue to test everyone again next week. The facility has been working with Los Angeles County Department of Public Health on providing a scheduled through the window visitation for family members that we will make available by next week. Please know that these visitations are limited to specific days and times. August 18, 2020
Yesterday an employee informed us that they tested positive for COVID-19. The employee is following quarantine guidelines. We are continuing to monitor our residents and staff for any signs and symptoms. We started mass testing for all our residents and staff today.
August 7, 2020
We received all our residents’ test results from last week’s testing and all of the results were negative. We are still waiting for a few more staff test results but so far all the results we have received have been negative.
Our facility continues to work closely with the local health department and will provide you with any updates.

As we continue our response driven testing of our residents and staff, there have been no further positive covid results from our testing last week. We continue to work with our local health department for guidance regarding our current situation.
July 24, 2020
We have learned that 1 resident has tested positive for COVID-19. As such, we are continuing to work with Public Health and various local agencies to stop the spread of COVID-19.
July 23, 2020
The facility has been in rapid response mode. As such, the facility is continuing to receive the test results of both residents and staff. The facility has learned that 2 staff members have tested positive. They are now under quarantine. The facility continues to collaborate with Public Health in the fight against COVID-19.

As we continue our response testing, no new cases of COVID 19 have been detected as of the writing of this update. We are continuing our weekly testing and daily monitoring of residents and staff and will follow guidance from the local health department. July 9, 2020
We have now learned that one of our employees tested positive for COVID-19. The employee is under quarantine at home. The facility has not converted to response testing mode. The facility will continue to work with the Los Angeles County Department of Public Health. We will provide updates as they become available.

We continue to test and monitor everyone at the facility (residents and staff) for COVID 19. We received our test results this week and as of today no one is testing positive. The facility has not had any new COVID cases. However, in an effort to stop the spread, the facility will continue to test its residents and staff weekly. We will continue to provide updates with any new development.

This week we continue with response-driven testing for our residents and staff due to the employee who tested positive in the beginning of this month. So far all the test results that have come back from this week and last week’s testing have had negative results. We continue to use best practices and guidance from Los Angeles County Department of Health to care for our residents. Thank you for your understanding during this difficult time.

This week we continued to do testing for our residents and staff members. We have not received the results yet from the testing efforts. We continue to monitor residents and staff daily and are pleased to report that we have not had anyone symptomatic of COVID 19. As we mentioned last week we will be updating you regarding the results from the facility testing that was done last week for the sample of residents. We are pleased to announce that none of these residents tested positive. Today we conducted a mass testing of all our residents and had our employees go to testing centers. We will be reaching out to you again once we have collected all our results. We continue to limit visitation to essential workers and visitors only, we monitor and surveil everyone that comes through to our facility and work with the Los Angeles County Department of Health to mitigate the situation. The results for the COVID test that was conducted for a sampling of our residents have not been reported back to us at the time of this update. We will report on the results of those next week once we receive them. Currently no residents or staff are exhibiting any signs and symptoms for COVID-19. We will once again conduct mass testing of all our residents and staff next week as part of our strategy to monitor and surveil our COVID Status. We will continue to work with Los Angeles County Department of Public Health.
June 3, 2020
During our COVID-19 surveillance testing this week one staff member tested positive. The staff member is currently on quarantine at home and is following Los Angeles County Department of Public Health’s return to work guidance. The sample of residents that were tested last week all yielded negative results. We continued testing a sample of residents this week. We will inform you about the results of those tests as they come. We will continue to follow guidance from LACDPH.
May 29, 2020
This week we tested a 25% random sample of residents for COVID-19. We have not yet received the results for these tests but will update next week or earlier. Our employees continue to re-test using LA County testing sites. Results from last week’s tests are continuing to come in. So far all re-test results for our staff have come out negative with 97 out of 114. We continue to monitor our residents and staff and so far there are no observed residents or staff will COVID-19 signs and symptoms at this moment. Please stay posted and or call our facility if you have any specific questions or update requests.
May 22, 2020
On May 18th, we re-tested all our residents per guidance from Los Angeles County Department of Public Health (LACDPH). All residents tested negative. Our staff conducted off-site testing and their test results are starting to trickle in. So far all staff results that have been reported to us are negative. We will provide further update on this when we receive all the results. The facility continues to monitor residents and staff daily for signs and symptoms of COVID-19. No one has been observed to have any new signs and symptoms at this moment. Please visit again for more updates as they come in.

Since the facility’s last update of May 8, 2020, there has been no new cases of COVID-19 or any residents or employees exhibiting any respiratory symptoms related to COVID-19. In the upcoming week, the facility will be conducting another mass testing of its residents and staff alike. The facility continues to implement and adhere to all the recommended safety precautions.

This past week, we were approved to do mass testing for Covid-19. We were able to test all residents and staff. Today we learned that one of our residents tested positive for Covid-19. That resident is in a private room under strict isolation precautions. No staff tested positive. No other residents or staff are showing signs and symptoms today.
Know that we continue to monitor all staff members before and after each shift for signs and symptoms of Covid-19, including taking each person’s temperature. We are adhering to the infection control guidelines as outlined by public health.
